Learning Disabilities Community Services
A dynamic person centred service that responds to the needs of individuals
Community Services provides support to people with a learning disability and/or physical disability in their own home, group setting or with parents and carers.
We are an accredited provider for Bristol and South Gloucestershire Social Services and are a registered Domiciliary Care Agency with the Commission for Social Care Inspection. We also provide services funded through Supporting People and The Independent Living Fund.
Person Centred approaches enable creativity and increased understanding of the ways to plan and work with a person with a learning or physical disability and is central in developing people’s capacity to make there own lifestyle choices.
Service user involvement is fundamental to our person centred work and we focus on self empowerment. Aspects have a committed programme of user evaluation and user focus groups to help shape policies and standards of care.
How the Service Works
Referrals are accepted from any location or profession and we also welcome self referrals. Upon application, we jointly undertake a person centred assessment in order to meet any needs identified. Our support service is user led and following an assessment, service users develop an Individual Support Plan to reflect their own unique needs and goals. Community Services support workers provide ongoing support. There is no time limit to the services we provide, but services are dependent on the persons funding.
Hours of Operation
We can provide support services seven days a week including evening and night support. We also operate an out of hours telephone support and advice line for our support workers.
